Welcome to the Provo City Library! We hope you enjoy the resources accessible to you with this card. 

TERMS & CONDITIONS 

I hereby agree to obey all rules and reulations of the Provo City Library:

  • I agree to return library materials when they are due.
  • I agree to pay replacement cost for any materials I fail to return.
  • I am responsible for all fines, damages, losses and collection costs charged against me.
  • I agree to pay any charges on this card even if the card has been lost or stolen until it is reported.
  • I will report any changes in my address, telephone number, or email address immediately.
  • I understand that my account will become inactive if not used for 2 years.
  • There will be a $1.00 replacement charge for a lost or stolen card.

Notifications: You are selecting your method of notifications. Please update your phone and/or email address when changes occur. If you fail to receive reminders or notices, you are still responsible for returning items on time.

The Library and its agents will use the personal informaion you provide solely to manage your account. The Library and its agents do no sell its list of account holders to orther organizations or groups.

TÉRMINOS Y CONDICIONES

Estoy de acuerdo a obedecer las reglas y regulaciones de Provo City Library:

  • Retornaré los materiales de la Biblioteca a tiempo.
  • Pagaré el costo por los materiales que no he regresado.
  • Seré responsable por multas, daños, pérdidas y colecciones de costo encontra de mi.
  • Acepto pagar cualquier multa en esta tarjeta incluso si la tarjeta se ha perdido o ha sido robada hasta que se informe a la biblioteca.
  • Reportaré algún cambio de dirección o número de teléfono a la biblioteca.
  • Entiendo que mi cuenta quedará inactiva si no la uso durante dos años.
  • Entiendo que habrá un cargo de reemplazo de $1.00 por una tarjeta perdida o robada.

Notificacións: Usted escoja el método de notificaciones. Por favor, actualice su dirección de correo electrónico, o número telefónico, cuando hay cambios. Si usted no reciba noticias ni recordatorios, aun es responsable de devolver materiales a tiempo.

La Biblioteca y sus agentes usarán su información personal solamente para administrar su cuenta. La Biblioteca y sus agentes no venden la lista de las cuentas de nuestros patrocinadores a otras organizaciones o grupos.
